Information submitted
The evaluation form may collect contact name, company, email, phone number, pickup ZIP code, transformer information, site-access notes, project timing and equipment photographs. Submit only information you are authorized to share.
How information is used
Submitted information is used to review the equipment, respond to the request, clarify project details, evaluate purchase or service options, coordinate accepted work and improve the intake process. Information may be shared with service participants when needed to evaluate or perform an accepted project.
Storage and security
Form records and uploaded images are stored using managed application and file-storage services. No internet service can guarantee absolute security. Do not upload confidential personal records, passwords, financial account numbers or unrelated sensitive information.
